TRT Process & Getting Started FAQ
5 questions answered about process & getting started for testosterone replacement therapy.
How do I start TRT online?
To start TRT online: choose a reputable telehealth TRT clinic, complete their health assessment, get bloodwork (many offer at-home kits), have a video consultation with a licensed physician, and if you qualify, your medication ships to your door. The entire process takes 1-2 weeks from assessment to receiving medication.
How long does TRT take to work?
TRT effects appear on a timeline: energy and mood improvements in 2-4 weeks, libido improvements in 3-6 weeks, body composition changes in 8-12 weeks, and full effects on muscle mass, bone density, and metabolic health in 6-12 months. Most men notice significant improvement within the first 4-8 weeks.
How often do you inject testosterone?
Most TRT protocols use weekly or twice-weekly testosterone injections. Some clinics prescribe every-other-week injections, but this often causes hormonal peaks and troughs. More frequent injections (every 3.5 days or daily microdosing) provide more stable blood levels and fewer side effects.
Should I inject testosterone subcutaneously or intramuscularly?
Both subcutaneous (SubQ) and intramuscular (IM) injections are effective for TRT. SubQ uses smaller needles (27-30 gauge), is less painful, and may provide more stable absorption. IM injections deliver larger volumes more easily. Most modern TRT clinics now recommend SubQ with insulin syringes for convenience.
How often do you need bloodwork on TRT?
You need bloodwork at baseline before starting, at 6-8 weeks to check initial response, then every 3-6 months for the first year. After stabilizing, most clinics recommend bloodwork every 6-12 months. Panels should include total/free testosterone, estradiol, CBC, metabolic panel, lipids, and PSA.
